I believe if your car is 1995 or older the CEL being on doesn't matter for inspection. OBDII 1996-1999 can pass with one fault code stored as long as it passes the tailpipe test. For EGR it is a visual inspection that the EGR components are there.
They seem to frequently tell me it needs new wiper blades in order to pass so I usually I replace them before bringing it.

Picky enough to make sure the car is "safe". There isn't much on there that isn't about safety but some shops always find something to pay for the inspection as they can't charge what it actually costs to do it.

Yes, PA safety inspections are quite strict -- one of the strictest in the country. However, it's all stuff that you really SHOULD make certain is in good order for safety's sake. Another thing that you'll find is that certain inspection stations can be much more strict than others. You can sometimes "shop" inspection stations until one passes your car.

As long as your brakes, tires, and suspension are in good order, you don't have much to worry about. My SVX has passed inspection every year without needing a thing, but I'm pretty proactive with regard to safety issues.

My suspension is ok. It's not horrible, but I can't have anyone sit in the back seat or anything heavy in the trunk. Everything else is fine.

Like Mike and Ben mentioned, inspections shops vary greatly. Generally the chains will be picky and look to boost heir profits by failing for wiper blades or cloudy headlights. Small shops may be concerned that you are "undercover" working for the state. I've had the best luck at small shops that know me before hand.
